The Dementia Care Program offers hundreds, if not thousands, of different group and individual activities, all with the goal in mind to help our clients.
Activities can range from games to gardening, music to finger painting! Here at The Remembrance Collaborative we have state of the art equipment, expert activities aides, and use evidence-based approaches in all that we do.
We staff 1 activity aide and two personal care, home health, or certified nurse aides for every 15 residents, bringing the grand total to 1:5, which is beyond industry leading standards, and far exceeds requirements of New York State.
Come see what we have to offer!
While your loved one is in our care, rest assured that we provide excellent quality of care in any scenario. Our expertly trained staff can assist with toileting, eating, dressing, grooming, and much more!
We staff two personal care, home health, and Certified nursing aides, along with an activity aide for every 15 residents, bringing the grand total to 1:5, which is not only leading the industry, but giving you peace of mind that your loved one will be cared for.
Here at The Remembrance Collaborative we offer case management and care coordination services at no cost. These services can include arranging transportation to and from the Dementia Care Program, or even linkage to the Office of the Aging in your county. Rest assured, like all of our staff, our case manager is also Dementia Certified through the Alzheimers Association!
Dozens of peer-reviewed studies have come to the conclusion that individuals with Dementia, cognitive impairments, or Alzheimers benefit tremendously from working with an occupational therapist. It can extend the amount of time a person may be independent and also allows for better quality of time and in some studies, showed an increase in memory capacity.
That is why we choose to have an occupational therapist on staff each and every day. No other program in the area has the infrastructure or team that we have to ensure that your loved one gets the most they can out of the Dementia Care Program!
Our team of experts came together, along with important guidelines from the New York State Department of Health and The Alzheimers Association to craft our safety initiatives. Delayed egress doors, window and door alarm bells, security cameras recording live in every common area- we have it covered. Additionally, our staff are all extensively trained on fall prevention measures and it is something we take very seriously. Your loved ones are safe here.
Additionally, each and every one of our expertly trained staff- from our activity aides to our nursing assistants are all Dementia Certified through The Alzheimers Association's rigorous curriculum. This training comes in handy when a client may have some emotional dysregulation, behaviors that may be harmful to themselves or others, basically every scenario one could think of- our staff are prepared for it.
