Memory Care Day Program Admissions

If you are interested in utilizing our Memory Care Day Program, please call or text Richelle at 716-432-8740. She is the non profit's President.

We will schedule for you to come and tour our location, so that you can see the ins and outs. We will give you an application packet to fill out while you're with us. A case manager will meet with you and make copies of your insurance cards in the case that you need assistance coordinating transportation.

Whether you'd like to reserve your time slot on a regular basis, or choose a few days ahead of time, it's up to you! 

We will create an ISP, or an Individualized Service Plan for you before your first day, so you'll just have to sign it when you come in :)
